A taxpayer's "ID Number Request Form" is the W-9 form.

The W9 or W-9 form is a "Request for Taxpayer Identification Number and Certification".

This is what a company or a person will fill out to provide official identification to someone who will pay them money for work or interest or any income as a self employed individual as well as for a few other situations.

The form also asks for your form of ID whether is your Social Security number or your Employer ID number.

You have to sign it and date it. You can be charged with perjury if you put false information on the form.

The general purpose is to provide the IRS with information about your income.

These folks would fill out a W9 form.
  • Individual or a sole proprietor or

  • C Corporation

  • S Corporation

  • Partnership

  • Trust Estate

  • Limited Liability Company

That is for things that are not covered on a W-2 form, such as interest, money paid for work as an individual or sole proprietor, money from real-estate transactions, cancellation of debt, money contributed to an IRS etc.

The form has four pages.
